Provider Information You have to have the insurance company added to the Insurance Company Lookup listing before you can proceed with this form. If you haven't yet done so, click here for instructions as to how to proceed. Adding your provider information.
Click "Insurance Company Lookup" off of the Administrative menu. Click on any field to jump to the explanation of that field. Click the You can either edit the entries you've received with the CILe installation, or you can add your own. To add your own, go to the row at the very bottom of the ones listed (where the star * is) and type in the information. Once you've added in the Insurance Company information, go here to add your provider information. The following are descriptions of each element on the screen: |
1) Insurance Company screen

Allows you to add your specific codes and numbers that uniquely identify you to the insurance company. |
2) Insurance
Company Name ![]() |
The common name by which you know the insurance company. You cannot add an insurance company here. You can only select it from the pull-down list. If your insurance company isn't listed, then go here for information as to how to proceed. |

Credentialled? ![]() |
Check this box for each insurance company
for which you are a full participating provider. This is for documentation
only and doesn't affect how the claims are submitted. |
4)
Assign? ![]() |
Check this box for each insurance company where you want the remittance check to be mailed directly to you. On this form, this is currently only for documentation, although I anticipate using it at a later date. For now, each session note has the same checkbox on it, so if you want the remittance check mailed to you rather than to the client, you need to make sure this box is checked on the session note screen. |
5)Tax ID

Typically, this will be your Social Security
number. If you also have an EIN (Employee Identification Number) from the
Federal Government, then you will need to be careful to put whichever of
those two numbers you submitted as your tax id to the insurance company
when you applied for credentialling. If this is wrong here, the insurance
company will not know who you are and will reject the claim. |

Tax ID Type ![]() |
Enter in a "24" if you are using
the EIN and a "34" if it is your Social Security number. You can
see that note at the bottom of the screen as a reminder when you click inside
the field. |

Provider ID ![]() |
This is the number that your insurance company
sent you as your unique identifier in their system. This number is in the
process of being replaced by the NPI with the insurance companies, so this
field will eventually be eliminated. |

Select the item off of the drop-down listing
which best describes the number you put in the "Provider ID" field. |

Reserved ![]() |
If you look at the CMS-1500 (HCFA) form,
you will see block 24J of the 08-05 version. It is called "Rendering
Provider ID #." Some insurance companies require the personal NPI,
some the full legacy ID, and others do not require anything. You will need
to refer to your insurance company's procedures to determine which number
belongs in this field. |

NPI Personal ![]() |
Your personal National Provider Identification
number. If you don't have one, go here
for information and to apply. An NPI is required to submit claims, so if
you don't have one, stop what you're doing and get it now. |

NPI Facility ![]() |
This is your group NPI, if you have one.
You may have one if you chose to have one as a personal NPI, and another
to cover you if you are a facility with multiple therapists or you may interested
in doing so some day. If you don't have a group NPI, then put your personal
NPI here. |

Ins Group / Billing ID ![]() |
This is the 'group' portion of the provider
identification number the insurance company gave you. This is in the process
of being replaced by the personal NPI, so this field will eventually be
eliminated, but for now, make sure it is accurate so the insurance company
can positively identify you. |

Ins Individual Legacy ID / Rendering ID ![]() |
This is typically the combination of the
'group' number you entered in the "Ins Group / Billing ID" field
plus the number that uniquely identifies you as an individual provider within
that group. If you are a single person office, then you will often have
"0001" as your personal rendering number (the word 'rendering'
refers to the actual therapist rendering the service). You will get this
number off of the paperwork your insurance company gives you when you get
credentialled, or if you are submitting claims out of network, it will typically
be your social security number. This number will typically be the group
number first, following by your rendering number. You can see some examples
above. |
