Intake


What is on the Intake screen?

The Intake screen allows you to collect biosocial information and other information that would be collected in the first interview. It includes the Mental Status Exam as well as a button that allows you to document the initial diagnosis. Completing the intake process officially "Opens" the client in CILe.

You access the Intake screen by right-clicking on the client's name and selecting "Intake" off of the client pull-down menu.

Note: Not all fields are explained here because they are largely self-explanatory. This help page is intended to introduce you to the concept of the "Intake" process and help you open your new client.



The following are descriptions of each element on the screen:

1) Intake Form This is the first therapeutic information that is captured by CILe. When you meet with the client for the first time, they are not yet 'Open' in CILe. By filling in this form, you are not only documenting the first session and the psychosocial information and mental status exam, but you are officially opening the client. Once you have completed this form, the client's status on their Fiscal Face Sheet will change to 'Open.'
2) Prior

The assumption is that you will not need to refer back to this form very often as you continue in your work with the client, so when you come into this form, you are presented with a blank form for new input. If you do want to see the client's last intake document, click this button.

3) Print This button allows you to view a printable copy of your intake document. Once you get the diagnosis done, it will also appear on the printed Intake report.
4) Diagnosis Once you have entered in the results of the session, click this button to document your diagnosis. This is an important step because this diagnosis will not only be printed on the printed report, but it will be the diagnostic code that is automatically entered into the session note when that is created. Without that diagnostic code, the insurance company will not pay for the session. You could enter it into the session note manually when you create the note, but using this feature is much easier and less likely to produce errors and questions. 5) Admission Date This is the date of the first session, which is the official date the client became a client. This is the date that goes onto the claim as the "date the illness began." For this reason, it is important that this date not be after the first session for which you are going to submit claims. It is also the date that shows up on the Fiscal Face Sheet as their Admission date.
6) Intake Worker This, of course, is the therapist. This is a left-over field from when this system was used for a multi-therapist clinic. CILe can no longer be used for this purpose.
7) Client Information Each input field is self-explanatory, so is not described in detail here. If you have questions, click inside the field, and look at the bottom of your screen, and a brief explanation is provided. If you would rather not fill in all of these fields, you are not required to do so. The only thing you need to do is enter in any one of the fields to cause the intake record to be created in CILe. When I don't want to do the entire form, I just fill in the "referred by" field. However, like I said in the "Diagosis" button explanation, don't skip the Diagnosis step.
