CILe - Client Information Ledger
Discharging a Client
Who should I discharge?

CILe has given you a quick way to determine if you have any clients who should be discharged. Click the "Therapy" menu and click the "Clients last appt" item toward the bottom of the menu. That will give you a listing of all of your open clients, sorted by the last date you actually saw them in session. The clients at the top of that listing may be candidates for discharge, depending on how long it has been since you last saw them.

How do I discharge a client?

Once you have determined that it is no longer necessary for this client to appear on your daily client listing and you do not plan on seeing them again soon, it is appropriate to formally discharge them. Filling out this form will document your final thoughts, and will change their status to "Closed" in CILe. You will still have access to all of their information via the "All Clients" listing that you can access from the "Open Clients" listing or the Administrative menu.

The Discharge Screen

You access the Discharge screen by right-clicking on the client's name and selecting "Discharge" off of the client pop-up menu.

Click on any field on the following image to jump to the explanation of that field. Click the arrow to return to the screen image. Note: Not all fields are explained here because they are largely self-explanatory. This help page is intended to introduce you to the concept of the "Discharge" process and help you close the client.

1) Discharge Form Allows you to document phone calls to or on behalf of a specific client. You can also document random notes that you want to make where the note is not as a result of a scheduled session. The note is documented with a Visit Code of "No Charge," which is how it will be documented on the client's "Attendance" screen. The "Change Incorrect Date" and "Add New Note" buttons work the same here as they do on the Session Note screen.
2) Diagnosis When you create the Discharge paperwork, you will need to associate a Diagnosis with it. To do that in CILe, click this button and display the Diagnosis screen. Use the navigation arrows to display the diagnosis you want associated with this discharge. This will typically be the last diagnosis you entered into the system, but if it is not, then just display the desired diagnosis. When you have displayed the diagnosis you want, then just exit out of the diagnosis screen by clicking the "X" in the upper right-hand corner. CILe will 'grab' the diagnosis and associate it with this discharge.
3) Prior The assumption is that you will not need to refer back to this form very often as you continue in your work with the client, so when you come into this form, you are presented with a blank form for new input. If you do want to see the client's last discharge document, click this button.
4) Print This button allows you to view a printable copy of your Discharge document.
5) Undo Discharge This button will allow you to reverse the Discharge process. You will have the option of either saving the Discharge Documentation or not, and it will change the client's status back to "Open."
6) Admission Date This is the date of the intake session, which is the official date the client became a client. This comes off of the intake document, and is not editable on this screen.
7) Discharge Date CILe will default this date to today, but you can change it to whatever you choose.
8) Problem at Intake CILe will automatically copy the original problem from the client's intake paperwork and insert it into the discharge at this location. This text cannot be changed.
9) Discharge Information Allows you to document your thoughts about this client. This may be the reason for a timely discharge, the client's prognosis for the future, or the fact that they simply stopped coming.
